Arizona law requires that all signers understand the document they are signing and sign willingly without coercion.
A diagnosis of dementia or memory impairment does not automatically prevent notarization. However, the signer must be able to:
- Clearly explain what the document is and why they are signing it
- Communicate directly with the notary without prompting from others
- Show understanding and willingness at the time of signing
If the signer appears confused, disoriented, or unable to demonstrate understanding, I am required by law to refuse notarization to protect their legal rights and ensure compliance with Arizona law.

⚠️ Note: Electronic submissions are not available at this time. We specialize in ink and roll or electronic captured physical fingerprint cards (FD-258), DPS, FBI, DLLC and others accepted by federal and Arizona agencies. EFT files can be created on a USB for ATF online application uploading.
Fingerprinting is a seperate fee and not included or provided by any applications you need to apply for.
Please research the requirements for your situation. Pay your application fees prior online and follow instructions via AZDPS , Liquor License, etc...

EFT files will be provided via secure email or USB
You’ll receive the completed fingerprint card to submit to your agency or licensing board along with their required forms. IVP fingerprint cards have to be sealed and mailed in by the fingerprinter and cannot be given back to you.
